It all starts with a chat, and there are multiple ways to get in touch with us.
You can call our main support line on 0330 058 3464, which is open Monday-Friday from 9:30am-4pm (except for Thursday afternoons when the line is currently closed). Or you can fill out one of our referrals forms here, after which we will call you.
We are happy to accept referrals from professionals, such as Social Workers and GPs, or from carers, family members or friends. If you would like to refer someone you support to our service then please ensure you have gained their consent before referring.
When we do speak to you, one of our team members will have a quick chat with you to understand your needs and circumstances. From here we will be able to provide one of three types of support:
Brief intervention support
If your needs are very straightforward then we can just ‘signpost’ you there and then. For example, by providing you with a telephone number you need or a timetable of classes at a nearby community centre
Referral to one of our Key Partners
Where appropriate we can refer you directly to one of our Key Partner organisations in order for you to get the specialist support you need (read more about our Key Partners here). To do this we will first ask for your consent as we will need to pass on some of your personal details to the organisation, who will then get in touch with you.
Person Centred Planning (PCP)
If you have a range of issues or complex needs that cannot be addressed through a quick chat then we will arrange for one of our dedicated Community Facilitators to call you for a more in-depth discussion. They will be able to offer specialist knowledge about services and activities in your particular neighbourhood and will help you to explore what changes you want to make. Because this is a longer process, we will need to take some personal details from you and add you to our PCP waiting list.
