Thank you for referring to Community Connections Lewisham. You are welcome to refer yourself, or you can refer another person provided they have given their consent for you to do so. Please answer the questions below to check if we are the right service to refer to. You can also check here.

There is currently a waiting time of around 8 weeks between submitting your referral form and our team getting in contact. If you require support more quickly, please call our phoneline as you will get help straight away
Community Connections Lewisham is not an emergency service so should not be contacted for urgent issues. If you’re worried about someone’s safety or welfare, contact Lewisham Adult Social Care on 0208 314 7777 or email them at Gateway@lewisham.gov.uk. In an emergency, always dial 999
