Wellbeing Plan

Who is it for?

      Any Lewisham resident (aged 18 or over) who needs in-depth support with exploring changes they’d like to make and accessing relevant groups and services that will support their health, wellbeing, and happiness.

What does it provide?

Up to 8 weeks of person-centred support from one of our Community Facilitators with specialist knowledge of their part of the borough. They will meet you face to face for an in-depth chat and work together with you to set some achievable goals and create an action plan. Follow up support includes sending you information, making appropriate referrals to specialist organisations, motivational phone support, and accompanying you on your first visit to a new group.

How do I refer?

You can refer yourself or someone you support (provided they have given you their consent) by phoning us on 0330 0583 464 or by completing our online referral form  N.B. If you feel your referral is urgent then we recommend phoning us directly as this will be the quickest way.

Which ‘Neighbourhood’ of Lewisham am I in?

The borough of Lewisham is comprised of four different ‘Neighbourhoods’. Once a person is referred for a Wellbeing Plan they will be contacted and supported by our Community Facilitator for their particular neighbourhood. They will be able to help you explore groups and services across the entire borough, yet will have specialist knowledge of your particular area.
