About Community Connections Lewisham
Community Connections Lewisham is a free Social Prescribing service, run by Age UK Lewisham and Southwark. We run a variety of services to help residents find and access the activities, groups, services and support that will improve their health, wellbeing and happiness. We can support all Lewisham residents, or those registered with a Lewisham GP, aged 18 or over.

Our Services
We run a range of different services, all of which are aimed to support residents to access their community.

Helping residents aged 18+ find the support, services, activities, and groups that will help improve their health, wellbeing and happiness.

Our volunteer drivers take residents to and from their destinations safely. Return journeys cost £10 within the borough.

Our volunteer Befrienders provide friendly weekly chats with residents who are unable to get out and about to local social activities.

Social Prescribing Supporting people aged 18+ affected by cancer to access the right services, activities, and community support that can improve their health, wellbeing, and quality of life.

Cancer Champions Empowering Lewisham residents to raise awareness and support their communities in accessing the services and resources they need to live well with and beyond cancer.
Our Community Work
We’re proud to be part of the thriving voluntary and charity sector in Lewisham. We support other groups, organisations, and charities around the borough to connect with one another, share information and learnings, and provide the best services for residents. We do this in the following ways…
Our Community Directory ~ our online Directory of information for professionals about the services, support, activities and groups available in borough – over 1,000 documents! To gain access to this and free training to use it, please get in touch.
Weekly Drop-In ~ Every Thursday morning (10:30am-12:30) you can find us at the Lewisham Irish Centre along with other partner services. Residents are welcome to pop in at any point and get information about support available. There are always refreshments and friendly activities taking place. Read more about our weekly drop-in.
Our Key Partners ~ We work closely with these borough-wide organisations to share information, learn from one another, and have quick and easy referral routes between our services. Our full list of Key Partners can be found here.
Social Prescribing Network ~ The Social Prescribing Network is an opportunity for all Social Prescribers and related professionals in the borough to meet, connect and learn about the services and support available. We run learning sessions, quarterly meetings and case discussion opportunities. Read more.
